• Copy of applicable official transcript(s)
  • High School (copy of high school transcript only)
  • GED (copy of final grade must also be attached)
  • College (copy of official transcript only)
• Copy of driver’s license (must include a certified copy of driving history if you possess or have possessed a driver’s license from any state other than North Carolina)
• Copy of military discharge papers (DD-214) (if applicable)
