IRS Form 1095-C

The City of Wilmington has completed and filed IRS Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, for the 2025 tax year.

A copy of your 2025 Form 1095-C is available upon request.

Accessing Your Form Online

You may access your 2025 Form 1095-C online by visiting: https://unum.benselect.com/enroll/Login.aspx?Path=wilmington

Login credentials:

  • User ID: Employer ID or full SSN
  • PIN: Last 4 of SSN + Last 2 of Birth Year

After logging in, select “Find a document or form” to retrieve your Form 1095-C.
